Where and how do you record decisions? How do you work with changes?

Solutions are documented in requirements management systems or documentation, such as Confluence, Jira, or specialized wikis. It is important that the document is accessible to all project participants and contains:

• description of the problem,
• solution options,
• the chosen solution with justification,
• responsible persons and deadlines.

When working with changes, I use change management approaches:

1. I record a change request with a description of the reason and impact.
2. I assess risks and resources for implementation.
3. I coordinate changes with the team and interested parties.
4. I make changes to the documentation and code.
5. I test and monitor the implementation.

This approach helps minimize errors and maintain transparency of the process.
