One of the biggest mistakes at my previous job was related to insufficient testing of changes in a critical module, which led to a production failure and service downtime for several hours. After that, I implemented more rigorous code review and automated testing processes, as well as set up monitoring and alerts to quickly respond to similar incidents in the future.
