Tell about a failed situation in a project and what went wrong.

In one of our projects, we faced a problem when new functionality was implemented without sufficient testing, leading to interface failures for users. The main issue was inadequate communication between the development and testing teams, as well as rushing the release.

As a result, users complained about incorrect data display and errors when interacting with forms. We conducted a retrospective, identified bottlenecks in the process, strengthened quality control, implemented automated testing, and improved communication between departments. This helped prevent similar situations in the future.
